Secondo giorno di terapia Ishida..
Ancora ovviamente non ci sono stati risultati, e Cirillo e' sempre piu' debole. Oltretutto ogni volta che ci avviciniamo per coccolarlo ha paura che stiamo per fargli qualche altro scherzetto..Al mattino ci sono le vitamine in pasta, l'antibiotico e il prednisolone, a pranzo c'e' l'interferone, nel pomeriggio le altre vitamine in perle e la sera la seconda dose di prednisolone e di antibiotico..
In realta' abbiamo entrambi paura che la malattia sia in stato gia' molto avanzato e che le speranze di salvare il nostro piccolo siano poche. Oggi non ha mangiato quasi nulla, lo imbocchiamo con il cucchiaio e non sempre accetta il cibo. L'acqua la cerca poco e si muove con molta difficolta'. Nel tardo pomeriggio abbiamo notato che aveva di nuovo le orecchie fredde, gli abbiamo misurato la temperatura e aveva solamente 36,9. Troppo bassa. Su consiglio della nostra amica allevatrice gli abbiamo dato subito acqua e zucchero e lo abbiamo coperto con un mio maglione di lana pesante. Dopo un po' le orecchie sono tornate piu' calde...
Oggi a un certo punto ho deciso che potevo fare ancora qualcosa per aiutarlo, e cosi' ho contattato direttamente il Dottor Takuo Ishida in persona. Si, sono pazza.
Gli ho scritto un'email illustrandogli tutta la situazione dai primi sintomi ad oggi e allegandogli tutta la documentazione possibile: scansione della lastra, risultati di tutti gli esami del sangue e del test PCR FCoV. 
Quando meno di due ore dopo ho trovato nella cartella posta arrivata un'email con mittente "Takuo Ishida" non ci credevo..
E' stato gentilissimo, una persona veramente adorabile. Ha letto tutta la documentazione che gli avevo fornito, ha osservato tutti i valori degli esami con attenzione e mi ha dato una vera e propria diagnosi a distanza. Ecco alcuni estratti:
"Ho letto la descrizione della malattia del tuo gatto e credo fermamente che si tratti di un caso di FIP umida. Il gatto ha la febbre, una leggera anemia, un elevato livello di globuline e liquido ascitico. Nonostante le caratteristiche del liquido ascitico non siano state descritte, se e' spesso, con un elevato valore proteico e un relativamente basso numero di cellule, dovrebbe trattarsi del tipo di fluido ascitico associato alla FIP. Oltretutto, il risultato positivo della PCR FCoV nel sangue o nel liquido ascitico supporta fermamente la diagnosi di FIP." 
Sono stata felice di aver letto anche alcune parole di conforto per la nostra amica allevatrice, che sta vivendo insieme a noi questo brutto momento. "Deve essere chiaro che nonostante possa esistere un'infezione FCoV nella sua abitazione, nessuno puo' biasimare l'allevatrice per la presenza del coronavirus nella colonia. Infatti il suo storico indica che l'allevamento e' eccellente e che i suoi gatti non vivono una situazione di stress. La FIP e' causata da una mutazione del coronavirus nel corpo del gatto. Questo emergere del virus e' solitamente associato allo stress nel gatto. Di conseguenza, e' ovvio che il tuo gatto ha subito un episodio di stress in qualche modo, anche se la sorgente del coronavirus rimane sconosciuta."
"Riguardo al trattamento, mi dispiace dirti che le possibilita' di successo nei gattini con FIP e' molto bassa. E' consigliabile non rimuovere troppo liquido ascitico dall'addome, a meno che il gatto abbia problemi respiratori o problemi a urinare. Il Dexamethasone puo' essere iniettato direttamente nella cavita' addominale a 1mg/kg una o piu' volte (con giorno di separazione tra un'iniezione e l'altra). Il prednisolone (non prednisone) puo' essere dato a 2mg/kg una volta al giorno (la sera) o due volte al giorno. I veterinari italiani tendono usare l'interferone per via orale a basso dosaggio, come 10U/kg ogni giorno per combatter l'infiammazione. Io uso questo regime per casi di stomatite FIV, ma non l'ho mai usato per la FIP. Io utilizzo sempre una dose iniziale di 1MU/kg sottocunatea un giorno si un giorno no fino a quando il liquido ascitico e' andato via, e poi usare lo stesso dosaggio una volta alla settimana come mantenimento. Gli steroidi orali possono essere ridotti gradualmente a 1mg/kg oppure 0,5 mg/kg."
Mi ha augurato che il trattamento abbia successo e che Cirillo migliori presto. 
E tutto questo mentre in Giappone era l'una di notte. 

Ishida therapy, day 2..
It's obviously too soon to see any positive results, and Cirillo is every minute weaker. Plus, every time we get close to pet him, he seems to be afraid that maybe we are going to make him swallow another disgusting medicine..In the morning we have vitamines #1, the antibiotic and the prednisolone; at lunch there is the interferon, in the afternoon vitamins #2 and in the evening the second round of antibiotic and prednisolone..
To be honest, we both fear the disease have already made a long step forward and there are very few chances of saving our little tiger. Today he almost didn't eat at all, we are feeding him with the spoon as if he were a baby, and sometimes he doesn't even accept the food. He's not particularly interested in drinking either, and visibly walks with difficulty.
During the afternoon we found his ears quite cold again, so we measured his temperature and it was 36,9 degrees. Way too low. Our friend the breeder suggested us to give him water with sugar and cover him with a wool sweater. Later his ears came back to normal....
Today at one point I decided I could do something more to help Cirillo, so I contacted directly Doctor Takuo Ishida. Yes, I'm crazy.
I wrote him an email, explaing the whole situation from the first symptoms to the way our kitten is today, and enclosing all the documents I had: the x-ray scan, all the blood test results and the PCR FCoVresponse.
When less than 2 hours later I found a new message in the mailbox, "from Takuo Ishida", I couldn't believe it..
He was so kind and helpful, really a lovely person. He had read all the documents I enclosed, all the response values and he gave me a real long distance diagnosis. Here are some exctracts:
"I have read the description of the disease of your cat, and I strongly believe that the disease is wet-type FIP. The cat has a fever, slight anemia, an elevated serum globulin level and ascites. Although the nature of ascites was not described, if it is thick with a high protein level, and with a relatively small number of cells, it should be the ascites associated wit FIP. Also, the positive result for FCoV PCR (although the sample was not specified) in blood or ascites strongly supports the diagnosis of FIP."
I was happy to read he spent a few kind words for our friend the breeder, who is living this bad moment veru close to us. "It should be reminded that nobody can blame this breeder for the presence of FCoV in the colony. The history indicates that the breeding there is excellent and that the cats obviously had no stress there. The FIP disease is caused by a FCoV mutant generated in the cat body. This emergence of a new virus is usually associated with stress in the cat. Therefore, it is obvious that your cat had stress in some way although the source of the original FCoV infection is unknown."
"With regard to the treatment, I am sorry to say that the success rate is very low in the young cats with FIP.  It is advisable not to remove much fluid from the abdomen unless the cat has respiratory difficulty or low urine production. Dexamethasone can be injected directly into the abdominal cavity at 1mg/kg once or several times (several days apart) . Oral prednisolone (not prednisone) can be given at 2mg/kg once a day (evening) or twice daily. Italian veterinarians tend to use oral interferon at a very low dose such as 10 U/kg daily to fight against inflammation. I use this regimen for FIV stomatitis cases, but have not used for FIP. I always use the initial dose of 1MU/kg subcutaneously every other day until ascites is gone, and then use the same dosage once every week to maintain the disease-free state. The oral steroid can be tapered to 1mg/kg or 0.5mg/kg gradually."
He wished me good luck with the treatment and that Cirillo goes into remission soon. 
And all this when in Japan was 1 AM.
